We use the information we collect about you to process appointments and to provide you with a more personalized experience. While you can visit the website without registering or providing any personal information, you will need to provide some personal information if you decide to register or request any of our services.

College Optical Express respect the confidential nature of personal information such as your name, credit card information, email and postal addresses, and telephone numbers. Because we must collect certain information from consumers to provide our assorted services, we have a fiduciary responsibility to protect that information from misuse. Any information provided by consumers is stored in a secure location, accessible only by our staff, and is used only for the specific purpose requiring the disclosure.

As a recorded visitor of our website, we may, in the future, occasionally call upon our visitors via email to provide input or respond to surveys related to our services. Our servers do not track, collect, or distribute personal information about visitors to our website. This information is used by our technical engineers to compile aggregate statistics about the total number of daily visitors to our site, the pages most frequently visited, and how long people stay at each page. None of the information that we report allows for the personal identification of individuals.

Medical records may be delivered to a primary care physician or any other physician that is directly or indirectly responsible for your medical care or the payment thereof.

This office will not use or disclose any of your medical and financial information for any purpose not stated above without your specific authorization. You may revoke your authorization at any time.

You may request restrictions on certain uses and disclosures. This office is not required to agree to a requested restriction. You have the right to receive confidential communications of your protected health information. You have the right to inspect, copy and amend your protected health information. You may also request an accounting of disclosures of your protected health information form this office.

We are legally obligated to maintain the privacy of your protected health information, to provide you with this Notice of Privacy Practices, and to abide by its terms. We reserve the right to change our privacy practices and apply revised privacy practices to protected health information.

You may register a complaint with this office if you suspect that your privacy rights have been violated. We will investigate the complaint and inform you of the findings. This office will make no retaliation against you because you registered a complaint. You may also file a complaint with the Secretary of the Department of Health and Human Services.
